EXP Credit Adjustment Approval Form - 

 

TO BE COMPLETED BY REQUESTING STUDENT AND THEIR EXP SPONSOR.

 

STUDENTS SHOULD CONSULT WITH, AND SEEK APPROVAL FROM, THEIR EXP SPONSOR PRIOR TO COMPLETING THIS FORM.

 

Experiential Education at the University of Kentucky is designed to assist in connecting students to academic credit and career-related learning opportunities in an internship, cooperative education (co-op), fellowship, or externship experiences. Students are able to gain credit for their experiences through the Stuckert Career Center with consultation with their Academic Advisor.
 
This form is used by the Office of Experiential Education to verify that a student has received the proper approval from their EXP Sponsor to change their credit hours.
 
Credit Adjustment Approval 
  1. Students must meet with their EXP Sponsor to process a course credit adjustment.
  2. Students must complete the EXP Credit Adjustment Approval Form.
    1. Student inputs the requested information, provides their EXP Sponsor contact email, and will digitally sign.
    2. Form will be automatically sent to the EXP Sponsor for their review and they will digitally sign.
  3. The Office of Experiential Education will review and submit request to the Office of the Registrar.
  4. If approved the student and the EXP Sponsor will recieve notification from the Office of Experiential Education.
  5. The Office of Experiential Education will adjust the credit hours in the students Handshake Learning Contract.
 
Academic Course Work Changes
Course credit adjustments will affect the academic course work a student will complete. If a student is approved for a course credit adjustment then a new course syllabus must be given to the Office of Experiential Education at expblue@uky.edu to reflect the academic course work changes.
 
Course Credit Descriptions 
EXP 396 is 1-12 credit hours per term. May be repeated to a maximum of 30 credits.
 
EXP 397 is only offered as 1 credit hour. Students will recieve full-time status.
 
EXP 455 is only offered as 3 credit hours. Students should consult the program coordinator for the Certificate in Entrepreneurship Studies at the University of Kentucky. Prereq: COM 381 and consent of instructor.
 
EXP 510 (International Service Learning) is 1-12 credit hours per term.
 
EXP 650 (Graduate) is 1-9 credit hours per term.
 
EXP 651 (Graduate) is 1-9 credit hours per term.
